The NTSB investigation’s official report determined that an act of sabotage caused a fatal mechanical failure in the helicopter’s tail rotor. Onboard data revealed it was not a high-impact crash, for the pilot attempted two emergency corrections to stabilize and keep control of the craft before colliding with the ground.
During the first of these corrections, the port passenger door had been mysteriously opened, which allowed Diana to be ejected through the door and then fall twenty feet to the ground. During the second correction, the pilot reduced airspeed and flattened the angle of the trajectory by approximately twenty percent, which reduced the force of the impact.
Forensics helped identify bodies in an aviation crash. But when the NTSB could not find any evidence of a body in the wreckage of John Herald’s crashed helicopter, the bureau requested help from the FBI’s forensics team. Diana Willis was the only confirmed survivor. The mystery of John Herald’s disappearance baffled experts from many agencies, including several from the intelligence sector.
The helicopter had struck at a flat angle, and the fuselage remained fairly intact as it rolled onto its side. At that point, the main rotors clawed the earth and tore themselves to shreds. However, the cockpit kept its integrity. The fuel cells ruptured, but there had been enough time for the pilot to escape before the helicopter fireballed. Since neither John Herald nor any evidence of his remains was found at the scene, the only conclusion was that he had either walked away or someone had recovered his body.
